When I sleep at home, I find that I sleep on my side more.  And I think that is what puts the torque on my back and makes me hurt in the morning.  I also have medical reasons, but the way I sleep doesn't help.

I have never used an air mattress when camping.  I just drop a sleeping bag and crash out.  But outside, I sleep on my back (because of my hips), and I sleep my better/  The earth is just soft enough for me.  

I try to sleep on my back at home, but I still wake up on my side and stiff

Hehe, I used to be all about just throwing a sleeping bag on the ground and crashing out there... then it had been a long time and in 2009 I started working at the Renaissance Faire and decided to skip the air mattress and go for the ground... big mistake.  I hadn't accounted for hips and absolutely could NOT sleep that night.  I'm a pillow top for life now. 

I also have back and hip pain but I find the opposite is true for me (pillow top works to help me stay somewhat lined up as apposed to the firm mattress my bf has.)  C'est la differences between men and women's body shapes.  :)

I got the pillow top so my girlfrend would be more comfortable, but not so soft that it would bother me.  I am the most comfortable and pain free when I camp and sleep on the earth

It's semi firm with a pillow top.  I have a bad back and joints, and from experience with soft mattresses, they make my pain worse.

Do you have a firm mattress?  If so, have you tried out a pillow top to see if maybe that will help with any sort of internal compression?

I get that.  I like to sleep facing the wall.  But when I lay on my right side for a long time I actually feel kind of nauseous.  I think it's how my organs are sitting.  Like if my stomach is compressed or floating makes a difference physically.  Even when I sleep facing the other end of my bed (where my feet normally are, I need to be on my left side

The trick that worked for me is, I noticed that I wasn't necessarily favoring one side over the other.  I was favoring facing the outside of the bed... so I flip-flopped so my head was at the footboard and was able to trick my body into sleeping on the opposite side of my body.  :P
