It's the last on the list, though the second and fourth refer to the same thing.
It's the number one cause of dandruff according to the most recent studies.

I never heard of that before. Maybe it is the cause in some instances, but it wouldn't be possible for all. http://www.medicalnewstoday.com/articles/191260.php
dandruff is caused by a fungus that lives on human skin.
after washing you want to finish by rinsing with cold water and then you want to get your hair dry as quickly as you can.
the fungus thrives in oily and warm areas of the body and heat increases the skin's production of oil, as does frequent washing.
it may help to only use soaps or acv/bs in the lock 1-2 times a week but to rinse hair w cold water every day in between.
might seem to not get better fo 2-3 weeks but that's how long it takes to teach body to reduce oil output to scalp
If you're concerned, then you can rinse out your hair in cool/cold water after the gym to get most of the sweat out every other time at the gym instead of washing it, that is to say, if you don't have high quality dread soap.

Had to email 'em to place my order, because ordering through the website resulted in an error. They were most accommodating and very helpful. It was no trouble at all to help me make sure that the order got sent my way.
I ordered 2 bars about a month ago, gave one to a friend who'd threatened to "steal" one of my locks so she could have the scent (and to make sure that it got used, not vacuum-packed and left in the freezer and forgotten... or eaten by the dogs.. who are convinced the tasty smelling hockey-puck sized cake is a big treat).
Anyway, a whole bar lasts me somewhere between three and five months.
I wash my hair ONLY with this shampoo, it's the only one I use.
I use it to wash my hair once to twice a week, depending on how messy it gets.
I plan to place a larger order later, as a number of my friends are falling in love with how my hair smells and how soft it feels, and the simple fact that I tell 'em they can use the shampoo and not "lock up" if they brush their hair and use a non-silicone based conditioner (preferably acholesterolone).
Dandruff "CAN" be caused by a fungal infection of the scalp.
That's why most anti-dandruff shampoos only work if you keep using 'em, because they only keep the fungus "under control", instead of removing it.
Dietary changes can have an effect on the health of the skin, and it can be useful to look into tea-tree, rosemary, and possibly an actual "anti-fungal" shampoo.
here's what I do when I'm supposed to be "presentable" for sake of others.
First thing in the morning, I go to the bathroom. After I wash my hands I don't towel dry them, I simply run my hands over my head. Long slow deliberate strokes from the forehead and back.
I then put a simple, lightweight bandana thing on, so that the hair dries being pushed back.
When it comes off my head about 10-20 minutes later because I'm ready to "go", the "mussed" look has become quite tame.
I hate it.
